Partial Clipping and Multilayered Wrapping Using Collagen Matrix for Partially Thrombosed Basilar Trunk Aneurysm: A Technical Case Report

چکیده
Treatment of basilar artery trunk aneurysms is still challenging today, although numerous approaches and modalities treatment exist. The authors present a case patient with partially thrombosed, ruptured aneurysm successfully treated by clipping occlusion the rupture point multilayered wrapping aneurysmal dome. A 49-year-old man presented to our emergency room chief complaint altered mental status. was diagnosed subarachnoid hemorrhage (SAH). No apparent bleeding identified on initial 3-dimension computerized tomography (CT) angiography digital subtraction (DSA). Follow-up DSA revealed thrombosed saccular emerging from trunk. We decided treat surgically partial including bleb via anterior transpetrosal approach. surgery performed without any complications, residual blood flow within diminished remarkably over time. Although direct for one most operations, it highly effective complex aneurysms, especially if other treatments are not available.

Despite the remarkable developments in neurosurgical and neuro-interventional procedures, the optimal treatment for large or giant partially thrombosed aneurysms with a mass effect remains controversial. Cerebral angiography through transfemoral approach is preferred in most of the situation though in cardiovascular surgery transradial angiography is accounted as the superior standard. The present study aimed at presenting our early experience of transradial approach to cerebral angiography and intervention of a patient with basilar tip aneurysm that had inappropriate vasculature to perform rou...

Dr. Eberval G. Figueiredo Rua Oscar Freire 1456 / 34 05409-010 São Paulo SP Brasil. E-mail: [email protected] Aneurysms of the anteroinferior cerebellar artery (AICA) are rare, comprising far less than 1% of all intracranial aneurysms. By 1989, only 33 cases had been reported in the literature. Subsequently, numerous case reports and small series of AICA aneurysms have been reported. Giant an...
متن کاملSimulation of the surgical manipulation involved in clipping a basilar artery aneurysm: concepts of virtual clipping. Technical note.
Despite recent advances in three-dimensional imaging based on a voxel-rendering method, these techniques do not simulate the morphological changes that occur during surgery. The authors' goal was to develop a computer-graphics model to simulate the manipulation that occurs during surgery when clipping a cerebral aneurysm. The authors developed an application program to interpolate the contours ...
